Abstract

Kraepelin proposed that schizophrenia and affective disorders were distinct disorders. This hypothesis has been challenged by proponents of the "unitary psychosis theory". In reviewing other alternative hypothesis, family data, symptom cluster data, boundaries and stability of diagnosis, the author suggests that there is no compelling evidence to indicate a common pathophysiology or distinct pathophysiology for schizophrenia and bipolar disorder. More problematic is a diagnosis of schizoaffective disorder. Additional research is required on the genetics of psychiatric illnesses or the pathophysiologic changes present in the syndromes currently described as schizophrenia and affective disorders.
